First Chiropractic Visit

Brief Paperwork

Upon entering the Rohnert Park Office, we will have some brief paperwork for you to fill out. The paperwork provides us with general information about yourself and your condition, and any insurance you would like us to check for you. For your convenience, you may download and fill out this paperwork prior to your appointment by clicking the appropriate links below. Simply download and print and bring the completed paperwork with you to your first appointment.


Consultation

Once the paperwork has been completed, you will have a consultation with the staff to discuss your health-related concerns, this is a great time to bring up things like Allergies, Back Pains, Whiplash or how to get more energy. This is also a great time to ask questions about the office and the staff.


History and Examination

In order to determine what the source of your problem is, the doctor will ask you various questions related to your condition. Next, a number of specialized Neurological tests will be performed to determine which tissues could be affected and to what extent.


X-ray Studies

Your specific condition may require us to take x-ray pictures of your spine to either rule out more serious conditions or assist us in developing the most effective treatment plan for you. This in-depth analyzing your films usually takes several hours. So, for your convenience, we will then schedule another time for you and your family to review the information the Doctor has gathered for you.


Home Instructions

Prior to leaving, patients may be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.


Schedule Your Next Appointment

You will be given a convenient appointment time for your follow-up visit. Generally speaking, patients are seen within 1-2 days and are provided with a complete review of the Doctor's findings. A Treatment Care Plan will be explained and payment options for your condition will also be discussed during this time, as well as answering your questions.
